Application Forms

University Student Placements

All placements will commence on Monday of your first week with Orientation Day.


Please organise travel so you arrive in Broken Hill by Sunday evening, ready to commence placement at 8.30am on Monday morning. Placements commence on Monday and mid-week starts are not permitted.

Accommodation is normally available after arrival on Sunday. If you require accommodation on the Saturday evening prior to commencing your placement, please note this in the comments box at the end of the application form. The BH UDRH will do its best to assist with your accommodation request, however all requests may not be possible.

On those weeks when Monday is a Public Holiday then Orientation runs on Tuesday and first day at placement is Wednesday of week 1.

Please complete an Application Form for all placement requests.

The New South Wales Department of Health requires all students to present their Immunisation Records to the Risk Management Unit for processing before a placement can commence. Please read . Immunisation requirements(pdf)

Please note that the student program will close on Friday 18th December 2009. All placements must be completed by this date. The Extended Clinical Placement Program close date is Friday 30th October 2009.



The program will shut down for four weeks over the Christmas break. There will be no accommodation available for students during this time.

The student program will re-commence on Monday 11th January 2010.
